Making an appointment

Making an appointment is simple. Call our office at (812) 754-1660 and ask to make an appointment. You will be asked for some basic demographic information (name, age, address, insurance, etc.).

WHAT TO EXPECT AT YOUR EVALUATION

At Samaritan Behavioral Healthcare, our goal is to provide the best quality care. For that reason, we attempt to do a thorough evaluation during your intake. You will need to arrive one hour in advance to complete the paper work or down oad the forms and do them at home (See forms below). Your initial evaluation will take approximately 45 to 60 minutes. During the interview the therapist will ask you questions about your symptoms, the history of your problem, and other relevant information. After the initial evaluation, you and your therapist will decide what would be the best course of treatment.

Samaritan Behavorial Healthcare accepts Medicare, Medicaid, and most major insurance plans. If you have questions, please feel free to call and talk to our office manager.
